About the Role

We are seeking a skilled and compassionate District Nurse to join our team through HCRG Staffing Connect. This role offers the opportunity to provide high-quality community-based nursing care, supporting patients to remain independent and receive care in their own homes while contributing to the delivery of excellent healthcare services within an NHS setting.

HCRG Staffing Connect is the app-enabled nursing agency, backed by HCRG Staffing. Our team of consultants and the app work together to enable nurses like you to take control of your work. Book shifts yourself, via a consultant, or use Auto-book to let us know when you want to work. The app will automatically pick up the shifts you want as soon as they’re published.

Key Responsibilities

  • Provide high-quality nursing care to patients within the community setting
  • Assess, plan, and deliver personalised care to meet patient needs
  • Manage wound care, medication administration, and long-term conditions
  • Monitor patient progress and escalate any changes in condition
  • Work collaboratively with healthcare professionals and community teams
  • Maintain accurate patient records and clinical documentation
  • Support patients and families with advice, education, and reassurance
  • Follow infection control, safeguarding, and patient safety procedures

Who We’re Looking For

Education: Nursing degree or equivalent qualification
Licences: Current NMC pin
Experience: 1 years of experience

  • Previous experience working within a community, district nursing, or primary care setting
  • Experience managing a varied caseload of patients with complex health needs
  • Confident working independently while delivering safe and effective patient care

Skills:

  • Strong clinical assessment and care planning skills
  • Ability to manage and prioritise a varied workload
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a multidisciplinary team
  • Compassionate, patient-centred approach to care
  • Strong organisational and decision-making skills
