Adult Social Worker – Community Mental Health Team
Salary: £34-£36 per hour
Location: Westmorland
HCRG Staffing is seeking experienced and motivated Adult Social Workers to join the Community Mental Health Team. This is an excellent opportunity to support vulnerable adults, promote independence, and work collaboratively within a multi-disciplinary setting.
You will play a vital role in assessing needs, safeguarding adults, and ensuring high-quality care planning across the region.
About the Role
You will conduct Care act assessments, social circumstances reports, hospital discharge, S117 Aftercare planning, support planning, reviews, safeguarding enquiries, signposting, multi-disciplinary working, crisis work, and hospital discharge tribunal work
Key Responsibilities
- Conduct Care Act 2014 assessments and strengths-based planning
- Complete Mental Capacity Assessments and contribute to Best Interest decisions
- Develop person-centred care and support plans that promote independence and wellbeing
- Work collaboratively with multi-disciplinary professionals across health and social care
- Support adults experiencing crisis, deterioration, or significant change in needs
- Lead safeguarding enquiries in line with statutory duties
- Maintain accurate, high-quality records, documentation, and reports
- Promote positive outcomes through relationship-focused and strengths-based practice
About You
We’re looking for dedicated practitioners with a passion for supporting adults to live safely, independently, and with dignity. You will bring:
- A Social Work qualification and Social Work England registration
- Experience in statutory adult services (mental health preferred)
- Strong knowledge of the Care Act 2014, Mental Capacity Act, and safeguarding procedures
- Excellent communication, assessment, and analytical skills
- The ability to work independently within a fast-paced, multi-disciplinary environment
- Confidence in managing complex cases and making evidence-based decisions
