Adult Mental Health Social Worker

Role: Mental Health Adult Social Worker

Salary: £34

Location: South Lakes and Eden

Job Type: Hybrid – 3 days in the office a week

Shifts: Mon – Fri; option to discuss condensed hours into 4 days a week

About the Role:

We are looking for a skilled Adult Social Worker who will provide essential support to individuals and families facing a wide range of challenges. You’ll assess needs, develop care plans, and work alongside multidisciplinary teams to safeguard vulnerable people and improve their quality of life. This is a highly rewarding role where you will directly impact the well-being and safety of those you support.

About you:

  • You will need to have a recognised professional Social Work qualification and be registered with Social Work England;
  • Have a good level of experience of working in a children and families setting;
  • Have a good knowledge of minimum standards and be able to achieve our expectation for promoting change in family life;
  • Be able to demonstrate the ability to analyse information and make informed decisions;
  • Good knowledge of child development.

Key responsibilities include:

  • Undertake Care Act assessments and produce comprehensive social circumstances reports

  • Coordinate hospital discharge planning to ensure safe and timely transitions

  • Lead and contribute to Section 117 Aftercare planning and reviews

  • Develop, implement and review personalised support plans

  • Manage safeguarding enquiries in line with statutory duties and best practice

  • Work collaboratively within multi-disciplinary teams to deliver integrated care

  • Provide crisis intervention and risk management support

  • Signpost individuals to appropriate services and community resources

  • Prepare and contribute to tribunal work and related statutory processes
